12. INFORMATION ITEM - Transforming Primary Care

  • Dr Amol Kelshiker - Non Pecuniary - Dr Amol Kelshiker declared a non-pecuniary interest in that he was a GP whose practice ran a walk in centre. He would remain in the room whilst the matter was considered and voted upon.
  • Dr Genevieve Small - Non Pecuniary - Dr Genevieve Small declared a non-pecuniary interest in that she was a GP whose practice ran a walk in centre. She would remain in the room whilst the matter was considered and voted upon.
  • Dr Shahla Ahmad - Non Pecuniary - Dr Shahla Ahmed declared a non-pecuniary interest in that she was a GP and Soar Beyond worked on a one to one basis with her practice. She would remain in the room whilst the matter was considered and voted upon.

8. Diabetes Strategy

  • Councillor Chris Mote - Non Pecuniary - Councillor Chris Mote declared a non pecuniary interest in that his daughter was a staff nurse at Northwick Park Hospital and that he had diabetic and kidney problems. He would remain in the room whilst the matters were considered and voted upon.
  • Councillor Sachin Shah - Non Pecuniary - Councillor Sachin Shah declared a non pecuniary interest in that his father is a National Council of Vanik Association committee member and that he was a volunteer. He would remain in the room whilst the matters were considered and voted upon.

  • Councillor Chris Mote - Non Pecuniary - Councillor Chris Mote declared a non pecuniary interest in that his daughter was a staff nurse at Northwick Park Hospital and that he had diabetic and kidney problems. He would remain in the room whilst the matters were considered and voted upon.
