Who is the Children’s Community Nursing Service for?
The Children’s Community Nursing team provide services to children and young people aged up to 16 years old and who have acute needs, or aged up to 19 years old and have complex needs. These children and young people require nursing care as part of:
- An acute short term care episode (such as iv’s, eczema, wounds)
- Long term care (oncology, CF, neurodisability, complex health needs, asthma and other respiratory conditions)
- End of life care
Children and young people who access this service must be registered with a Harrow GP and/or should be a Harrow resident.
What does the service provide?
The children’s community nursing team are registered children’s nurses who aim to provide a co-ordinated holistic approach that promotes safe practice, flexibility and the delivery of quality care to the child or young person. They work in partnership with children and young people, their families and other professionals in a range of community settings including home, school, nursery, playgroup and respite care facilities. This includes working in collaboration with the school nursing team to support the delivery of care, management of children’s nursing needs and provision of education and training within pre-school and schools settings.
How do I access the service?
Patients can access this service following a referral from a health practitioner. Following the initial referral, a named registered nurse will co-ordinate and oversee the nursing package of care based on the child or young person's needs. This will include working in partnership with families, individuals and carers to identify and meet the needs of the each group.
This service is available from Monday to Friday, 8am to 4pm.
