Who is the Denham Unit (Rehabilitation) for?
Adults aged 18 or over who are registered with a Harrow GP. They must have clearly identified rehabilitation goals and be able to engage in a rehabilitation process.
The service aims to prevent unnecessary acute hospital admission and facilitate overall shorter length of inpatient episode of care.
What does the service provide?
Denham Unit is a 30 bedded unit that offers rehabilitation services for a short period of time, so that all patients return to their own homes after a short period of care and therapy.
All patients are cared for in a single room with en-suite facilities and have access to a therapy team, including:
- Physiotherapists and occupational therapists (Monday to Friday)
- A pharmacist (Monday to Friday)
- A consultant geriatrician on call for the unit 24 hours a day
Three multidisciplinary ward rounds take place each week and the doctor follows patients from admission to discharge.
The unit has access to specialist nursing advice for wound care, diabetes, dietetics, speech and language therapy, and infection control.
Before most patients are discharged, they receive a home assessment and the therapy team work with social services to ensure a safe package of care on discharge.
Please note that the Denham Unit is not a dementia unit.
How do I access the service?
A patient requires a referral from a GP or STARRS (short term assessment, rehabilitation and reablement service) at Northwick Park Hospital.
The service is available 24 hours a day, seven days a week.
Visiting hours are between 2pm and 8pm.
The service is provided by London North West Healthcare NHS Trust (LNWHT).
