How to apply for a chaperone/matron licence
Application forms can be obtained from the Education Welfare Services (EWS). The form must be completed and returned to the EWS, and should be accompanied with two passport-sized photographs. An application form for a Criminal Record Bureau (CRB) disclosure must also be completed and returned by the applicant.
