This page contains a list of forms you may require for administering your pension. These would have been sent out to you when you commenced employment, however if your circumstances have changed you may be required to use these forms to notify us.
If you have opted out of the Local Government Pension Scheme and want to rejoin, please notify us in writing.
Form of Particulars Please note that you only have 12 months from joining Harrow to transfer in any previous pension rights to the LGPS. If you are interested in this option you can provide the details of your previous pension arrangement on the following form
Death Grant Nomination If you wish to nominate someone for your death grant please complete this form
Cohabiting Partner Form If you are married or in a registered civil partnership please do not complete this form as your husband, wife or civil partner is automatically covered for the survivor benefit in the event of your death
